 "NA Skills Check Off List: Please be sure that each box, under clinical practice has a date and your Initials.  You must write in a date and the RN must initials in the box that says RN initials.    Be sure there are no blanks.   I realize in the APNA class  with only two clinical days, this might not be a realistic goal.   Please do your best.  The students should have their skill checklist at clinical. We gave them a copy.  If they did not actually perform the skill in clinical you need to review that skill with the student/s and write the word "Verbalized" in each box.  There can be no blanks."~ NA Program Director Kristen Woods
